Children's Community Options Program (CCOP)
Children’s Community Options program provides supports and services to children living at home who have long term developmental, emotional, or physical disabilities. These disabilities must disrupt at least two functions of the child’s life. CCOP funding is aimed to provide supports to keep child in their home and community.
Eligibility Requirements
- Must be under the age of 22
- Be a resident of Wisconsin
- Be living in a home or community setting
- Require a level of care typically provided in an intermediate care facility

Children's Long Term Support Program (CLTS)
The Children’s Long Term Support Program is Home and Community Based Service waiver that provides Medicaid funding for children with substantial limitations in their daily activities and need support to remain in their home or community. Funding can be used to support a range of different services based on the assessment of needs of the child or the child’s family. Children with severe emotional disturbances, physical and or developmental disabilities may be eligible.
Eligibility Requirements
MUST MEET ALL REQUIREMENTS
- Under the age of 22
- Eligible for Wisconsin Medicaid
- Be a United States Citizen or have acceptable immigrant status
- Be a Wisconsin Resident
- Not have income in their name in excess of the current Medicaid standards
- Live at home or in a foster care setting
- Have a level of care need that is typically provided in an institutional setting such as a hospital, a nursing home, or an institution for people with developmental disabilities
- Be able to receive safe and appropriate care at home and/or in the community; and be able to receive safe and appropriate care at home and/or in the community that does not have a cost to the Wisconsin Medicaid Program that exceeds the cost Medicaid would pay if the child were in an institution.

Comprehensive Community Services (CCS)
CCS is a county level program for all individuals who need ongoing services for a mental illness, substance use disorder, or a dual diagnosis beyond occasional outpatient care, but less than intensive services provided in an inpatient program. Individuals in the program work with a team of service providers to develop a unique and personal treatment plan. Each person in the CCS program has a unique plan to best achieve success.

Community Recovery Services (CRS)
Community Recovery Services is a program that helps individuals living with mental illness designed to improve their quality of life, and reach their full potential. CRS includes three services; Community Living Supportive Services, Peer Support Services and Supported Employment Services. Community Living Support Services include activities that are intended to assure successful community living, such as personal hygiene, financial management, crisis coping skills and transportation. Peer Support Services include services assisted by an individual who has lived with mental illness, and is trained to help others on their journey to recovery. Supported Employment Services include activities to help participants locate and maintain employment.

Coordinated Services Team (CST)
The Coordinated Services Team is a county level program that is designed to develop a comprehensive, individualized system of care for children with complex behavioral health needs based upon the wraparound model. The Coordinated Services Team is a team made up of family members, service providers and others to develop and carry out a coordinated services plan for the child.
Eligibility Requirements
To be eligible the child must be involved in two or more systems of care, and other interventions must have been unsuccessful in the past due to a lack of coordination or scope of problems. The youth must be at risk of out of home placement, or transitioning home from out of home placement. Youth must also have private insurance and a mental health or substance abuse diagnosis. The parents must also be willing to be involved in the program.

Katie Beckett Program
Katie Beckett Program is a program that helps children with certain long term disabilities or mental health needs to receive a ForwardHealth Medicaid card. Children who are not eligible for Medicaid because of parent’s income or assets, may be eligible for Medicaid through the Katie Beckett program, even if the child is covered under a private health insurance policy.

- Be disabled by standards of the Social Security Act
- Be a United States citizen, or have acceptable immigration status
- Be a Wisconsin Resident
